What this means
When Milo goes up, they do not come down on their own. Arousal climbs fast, stays high for a long time, and while it is up there they are not hearing much. This is not disobedience: a dog at high arousal loses access to behaviours they know perfectly well, and saying the cue louder does not restore that access.
The most common mistake
Repeating the cue louder and louder while they are up. You get nothing in the moment, and you teach Milo that your voice is part of the background noise of interesting events. Wait for three seconds of quiet, then ask once.
